What is it?

It requires disclosure and management of any financial or personal interests that may affect a Covered Individual’s responsibilities at CUNY, particularly in research and sponsored projects.

– At the time of submitting a grant proposal or application.
– With annual progress reports.
– Within 30 days of any material change in disclosed interests or acquiring new ones.
– When new investigators join ongoing research projects at CUNY.
Required documents include:
– Significant Financial Interest Disclosure Form 1 [you are required to fill Form 2, only if you have answered ‘YES’ for any of the questions from 1 to 5 in Form 1]
– Grant or contract documents related to project funding
– Disclosure forms for intellectual property and licensing agreements if applicable
